However, when you choose a dentist in the large national network (National Options PPO 30 / UnitedHealthcare Dental), you receive network discounts without the hassle of price negotiations. Plus, there are no claim forms to fill out when you use a network provider.
• Growing Benefits: For basic and major services, the percentage the plan pays increases after your first year on the plan.

Choose from 3 Flexible Plan Options

Our plan options give you the freedom to choose what works best for your budget and anticipated dental care needs. All three plans feature a low $100 annual deductible per person, and all of them pay 100% (with no deductible) for preventive services, which includes 2 routine exams and cleanings per policy year.

• DentalWise Basic Plan: With an annual maximum of $1,000 per person, this plan is great for routine maintenance. It covers preventive services and basic services (like simple fillings and non-surgical extractions). Major services are not covered under this plan.
• DentalWise Plan 1000: This plan features an annual maximum of $1,000 per person. It covers preventive and basic services, plus it adds coverage for major services like bridges, crowns, dentures, and root canals. For major services, the plan pays 15% after your deductible in the first year, and 50% in the second year and after.
• DentalWise Plan 2000: Our highest value plan gives you double the annual maximum benefit ($2,000 per person). It covers everything in the 1000 plan, and it is the only plan that includes coverage for Implants. Implants are covered at 50% after a 12-month waiting period, and come with a separate $1,500 lifetime maximum benefit.
