EyeMed provides three distinct plan tiers in California, allowing for customized coverage based on how often you need new eyewear and your preferred budget.
- Bright Vision Plan: The premium option featuring the highest allowances, including a $200 frame or contact lens allowance.
- Bold Vision Plan: A balanced plan offering a $130 frame or contact lens allowance with standard copays for exams and lenses.
- Healthy Vision Plan: An exam-focused plan that provides a $0 copay for annual exams and significant discounts on frames and lenses.
Your EyeMed Benefits Summary (Advantage* Network)
The following benefits are available through in-network providers. All plans include an annual eye examination.
WellVision Exam & Health
- Eye Examination: Focuses on your vision and overall health. Available for a $10 copay (Bright/Bold) or $0 copay (Healthy) once per plan year.
- Retinal Imaging: Enhanced diagnostic screening available for a member cost of no more than $39.
Eyewear & Materials
Members can choose between glasses or contact lenses each plan year (unlimited frequency for the Healthy plan). What is included:
- Frames:
- Bright Plan: $0 copay with a $200 allowance, plus 20% off the balance over $200.
- Bold Plan: $0 copay with a $130 allowance, plus 20% off the balance over $130.
- Healthy Plan: 35% off the retail price for any available frame.
- Lenses: Standard plastic single vision, bifocal, and trifocal lenses are available for a $20 copay (Bright/Bold plans).
- Lens Enhancements: Members receive discounts on common upgrades, including UV treatment, tints, and scratch coatings (often $0 copay on Bright/Bold plans). Standard polycarbonate for children under 19 is covered at a $0 copay.
- Contact Lenses: Allowances start from $130 or $200 (depending on the plan) for conventional or disposable materials.
Additional Member Savings
- Additional Pairs: 40% off additional complete pairs of prescription eyeglasses at in-network providers.
- Non-Covered Items: 30% off items not explicitly covered by the plan.
- Laser Vision Correction: 15% off the retail price (or 5% off promotional price) for LASIK or PRK services through the U.S. Laser Network.
Note: Out-of-network coverage is available but results in lower reimbursement amounts. Residents in certain states (like Washington or Vermont) may have different discount availability.
