WellVision Exam

Focuses on your eyes and overall wellness. $15 copay. Routine retinal screening: up to $39

Prescription Glasses

$25 copay. What is included:

• Frames: $150 frame allowance; $170 Featured Frame Brands allowance; 20% savings on the amount over your allowance.
• Lenses: Single vision, lined bifocal, and lined trifocal lenses. Impact-resistant lenses for dependent children.
• Discounts on Lens Enhancements: Additional discounted fees are applied for Progressive lenses, Anti-glare coating, Light-reactive lenses, Tinted (colored) lenses, Impact-resistant lenses, UV protection, and other enhancement. Average savings of 30% on other lens enhancements. See VSP plan brochure for details.
• Contacts (Instead of Glasses): $150 allowance for contacts and contact lens exam (fitting and evaluation). 15% savings on a contact lens exam fitting and evaluation.

Additional Savings

• Glasses and Sunglasses: 20% savings on unlimited additional pairs of prescription or non-prescription glasses and sunglasses, including lens enhancements, from a VSP provider within 12 months of your last WellVision Exam.
• Laser Vision Correction: Average of 15% off the regular price or 5% off the promotional price; discounts only available from contracted facilities.
